If you would like to register online, please find the camp code of the event you want to sign up for and then proceed to Online Registration form at the bottom of this page. Then, please download and complete this Waiver And Release Of Liability form and mail it O'Sullivan Hockey or bring it with you to your first session. You will not be allowed to participate without this form.
